Abstract

Standardization and information reliability of health care record of the elderly are examined in case of coordinating health, medical and welfare services to elderly persons provided among municipality areas. In order to solve these problems, standardized health care record approach is examined in the four municipality fields. The results show the standardized elderly health care record approach will be applicable without major changes of the record formula, and most of all information of items of the record are reliable. The elderly health care record formula is already used in the data base information system of one municipality, and contributes to coordination of services.
