The free rectus abdominis musculocutaneous flap (free RA MC flap) can afford excellent results in autogenous reconstruction. However, despite the many advantages of this flap, its use involves a major surgical procedure that carries the risk of abdominal weakness, bulging, or hernia. Between 1995 and 2001, a total of 74 reconstructions were performed at our institution using free RA MC flaps. Sixty-one flaps were transferred for head and neck reconstruction, nine for trunk reconstruction, and four for reconstruction of the extremities. Of these cases, 60 were followed-up over 3 months. Among the 29 cases in which absorbable Dexon meshes were used, two cases experienced bulging, whereas there were five such cases among the 31 in which Dexon meshes were not used. There was no statistically significant difference between the two groups. However, in not a single one of these cases was there hernia, complaint of abdominal weakness, nor umbilical necrosis.
